How can I contact amazon and let them know I never received my order?

I ordered something from a 3rd party on amazon.com about a month ago, and I still have not received it. The UPS tracking doesn't it ever making to the next destination. Where is the contact form / phone # to tell Amazon.com that my order never arrived and I want a refund.

You can contact Amazon, by email form, phone or chat using their Contact Us form here:

If you use the Email form, someone at amazon should end up responding back to you within a couple hours.
